Today I learned that 2013 aviation accident Mozambican airline LAM, on a flight from Maputo to Luanda, was caused by the pilot deliberately crashing the plane, resulting in the deaths of 30+ people. This led airlines to implement a policy preventing the copilot from ever being alone in the cockpit
